Requirements

Experience formulating and updating care plansExperience working within drug and alcohol services or a similar support role.Home visits for clients with limited mobility to complete assessments and keywork.Home visits to clients who are off script/not in treatment to attempt re-engagement (usually carried out with another professional)Visits to service users in hospital to provide ongoing support (where appropriate)Onsite assessments, duty and clinic cover when requiredManage a small caseload of complex outreach client
